This form is used to withdrawal from a course (after the add drop deadline has passed).
Requirements
Your advisor’s signature is required.
If an athlete; your Coach’s signature is required.
If you have a Visa; your international advisor’s signature is required.
Additional Information
The deadline to submit is Friday of Week 8.
