Change of Advisor Form

Please discuss any requests, changes or petitions with the appropriate faculty member BEFORE submitting any forms. After the form is submitted it will be sent to the necessary faculty needed to complete the form. Please make sure the information provided is accurate. Please allow up to 5 business days before inquiring about the status of your request. You will receive an email of confirmation from a member of our office once the request has been processed.

**Any change of advisor form submitted during weeks 6-8 will not be processed until the beginning of week 9. You need to meet with your current advisor to get any holds lifted**

Use

Most students stay with their first advisor until they declare majors in winter of sophomore year and move to an advisor in the major, but students can switch advisors at any time by submitting a Change of Advisor form.

Requirements

New advisor’s signature is needed. The form will not be processed without this signature.

First-year students are required to get approval from the Director of Advising for advisor changes.

Additional Information

Advisor changes will not be processed from Weeks 6 – 8 each term.
